During the term of your subscription plan, you will have access to change or update your plan as often as you like.

What is long-term care?
"Long-term care" means helping people of any age with their medical needs or
daily activities over a long period of time. The care can be provided at home,
in the community, or in various types of facilities.
Why do I need a Lifestyle Care Plan?
No one can predict the future. You need to be prepared. A Lifestyle Care Plan
will give a caregiver the information needed to help in providing quality care.
The founders of the LCP have spent many years caring for people with long-term care
needs. All too often they have seen the negative effect on the quality of life
of individuals when caregivers do not have the information necessary to care
for them as unique individuals.
If I have a Lifestyle Care Plan, what good will it do me?
Home care agencies, nursing facilities, hospitals and other organized care
providers are required by Federal and or State laws to develop a written Care
Plan for everyone in their care. This Care Plan is also required by regulation
to be individualized and, every client is allowed by law to have personal
participation and input included in the development of the plan.
What if I never use my Plan?
We honestly hope you never need it. In some ways it's like auto insurance. You
really need to have it but, you never want to actually use it. Unlike auto
insurance, this is about protecting your most important asset, you!
I'm not sure I need it for myself but, I know someone in my family that
does. Can I do it or help them to do it?
The system is designed to accommodate this. You can help someone else or, you
can do it for them even if they are no longer able to actively provide their
assistance. If you have knowledge of them as a person, you should at least be
able to provide some information. Remember, even knowing some information is
better than nothing when providing care. Let's face it, if you have the opportunity to help a parent, spouse, child or
other loved one, isn't it something you really want to do?
What if I want to change any of my information?
You can add, change or delete any information at any time during your
subscription period. The system will automatically change your Plan based upon
any changes you have made.
There are some questions I don't want to answer, do I have to?
Except for a few basic questions at the beginning, the system does not require
you to answer any of the questions. Obviously, we believe the more complete and
detailed your responses are the better your Plan will be. But, that is entirely
up to you.
How long do I have to complete the Questionnaire?
You have the ability to complete your responses at any time during your
subscription service. The system will automatically update your Plan whenever
you add or change information during the subscription period.
What if the care provider doesn't want my Lifestyle Care Plan?
Any quality care provider will want to know as much about you as possible. If
they are not interested or don't care about your Plan, you really don't want
them to take care of you. Find someone else!
Why can't I just make a list of what I want?
You could but, is that really the best way? The Lifestyle Care Plan is a
carefully designed and comprehensive set of questions designed to produce a
very personalized plan that is formatted in way that is familiar to caregivers.
How will anyone know I have a Lifestyle Care Plan?
First, you will have a paper copy of your plan which you may give to anyone you
deem appropriate. We suggest that you give copies to family members, trusted
friends, your attorney, or anyone that may be in position to assist you should
the need arise. Your secure access code also allows a healthcare provider to
retrieve your plan via the internet should you choose to provide them, or
anyone else, with the code. The LCP system also allows you to e-mail copies of your plan to anyone you
Can anyone else look at or obtain my information?
No. Only those you give access to.
Is this a legal document?
Yes. It is our recommendation that you have your Lifestyle Care Plan notarized.
